Chronic Disease Management
The Chronic Disease Management (CDM) programme is designed to support the health and wellbeing of patients with specific chronic diseases.
This programme is free of charge for qualifying patients. To qualify for this programme patients must:
- Have a medical card or a GP visit card
- Aged 18 or over
- Have one of the following specific chronic diseases
- Chronic Obstructive Pulmonary Disease (COPD) - HSE Patient Information
- Asthma - HSE Patient Information
- Type 2 Diabetes - HSE Patient Information
- Cardiovascular Disease including; Heart Failure, Stroke, Atrial Fibrillation (irregular heartbeat), Ischaemic Heart Disease (Angina, Heart Attack)
The CDM programme provides eligible patients with two visits every 12-months with your GP or practice nurse. These reviews support you to manage your condition closer to your home, help with early detection of any new conditions you may develop or provide early detection of complications in your condition.
A personalised care plan is developed and agreed with you and reviewed along with your medication during your visits. During these visits you will also be provided with education and self-management support which plays a vital role in helping to improve your understanding of the chronic disease leading to improved patient outcomes through better condition management.
